SCUBA TRAINING INSTRUCTIONAL AGREEMENT Welcome to your SSI/PADI/ANDI training program at UnderWater World Scuba/Sports. Diver training courses are meant to be fun, but, like any form of learning, you'll need to invest some time and effort in reading and studying. This Learning Agreement is between you, the student, and UnderWater World Scuba/Sports' instructional staff, and covers our mutual responsibilities in this program. STUDENT RESPONSIBILITIES: The student agrees to study independently as specified by the instructor. In general, this means that before each class session, the student will: 1) Watch the appropriate assigned portion(s) of the course video(s). 2) Read the appropriate assigned section(s) of the course manual(s), and complete the exercise questions within. 3) Complete the appropriate Study Guide Questions/Knowledge Review(s) to be handed in at the start of class, or prior to the Open Water Check-Out dives, as specified by your Instructor(s). In addition, the student agrees to: 1) Follow all course procedures as set forth by the Instructor(s). 2) Ask questions about any class-related items not understood. 3) Show up for all sessions on time and prepared. If the student arrives at class, but has failed to complete and turn in assigned work, or if the student fails to arrive on time, it may be necessary to make up the work and continue the class at a later date. The student will be responsible for any additional costs and/or inconvenience this may cause. In scheduling and determining additional costs, UnderWater World Scuba/Sports will give every reasonable consideration to unforeseen events, such as family emergencies, that led to this situation. INSTRUCTOR/STAFF RESPONSIBILITIES: The course Instructor(s) and staff agree to: 1) Be prepared and ready to begin the course as scheduled. 2) Provide a positive learning environment in which to master the course objectives. 3) Answer the student's questions to the best of their ability. 4) Assist the student through learning challenges. If the student completes all course work as assigned, arrives for class promptly, and otherwise follows directions for learning as given by the Instructor(s), UnderWater World Scuba/Sports will accept responsibility for reasonable learning challenges. Course objectives must be met before certification before the student is certified, but in this situation UnderWater World Scuba/Sports will schedule any necessary training sessions, at no additional charge to the student, until either (A) the student masters the course objectives, or, (B) the student voluntarily withdraws. GENERAL TERMS AND CONDITIONS: 1) Your non-refundable deposit reserves your place in the above course. Balance of course fee is due the first class meeting. 2) Our policy on drugs and alcohol is simple - ZERO TOLERANCE during all dive related activities! 3) For all OPEN WATER training, the Air Delivery System, Air Management and Information System and Buoyancy Control System will be provided in the course fee, however Students will need to furnish the following: -Snorkel System (mask, snorkel, fins, booties) -Exposure System (suitable and appropriate for the conditions likely to be encountered) (NOTE: WE DO NOT HAVE EXPOSURE PROTECTION AVAILABLE FOR RENT!) -An underwater timing device and a slate to record pertinent critical information relating to time spent under the surface OR an approved Dive Computer. -Total Dive Log System (To log and verify all levels of training and to be signed off by both Student and Instructor(s). 4) Candidates are expected to furnish their own Total Diving System for use during any training above that of OPEN WATER program. 5) Any equipment NOT purchased from UnderWater World Scuba Sports MUST be reviewed and inspected by your Instructor(s) for safety and acceptability prior to it's use in any training. Rental equipment is available upon request at a special student rate for currently enrolled students in good standing. 6) The Open Water training program DOES include all course materials in the course fee. A packet will be issued to each student which contains: Text Book, VHS Video or DVD, Recreational Dive Tables and Student Training Record File Folder, which must be completely filled out by every student. The Certification Card, to be awarded at the successful completion of all requirements is also included in this fee. Also, DAN membership and insurance or equivalent, while not included, is highly recommended for ALL students.

REFUND POLICY:

A Full refund will be given up to one week before the beginning of MODULE
ONE. (STUDENT agrees to return all course materials and/or gear supplied by DIVE
INSTRUCTOR, providing they are in new condition. Retail price of used or damaged course materials and/or gear
will be deducted from refund.)

75% refund will be given up the beginning of MODULE TWO. (STUDENT agrees
to return all course materials and/or gear supplied by DIVE INSTRUCTOR,
providing they are in new condition. Retail

40% refund will be given up to the beginning of MODULE THREE. (STUDENT
agrees to keep all course materials supplied by DIVE INSTRUCTOR.) -
No refund will be given after the beginning of MODULE THREE.
Student SNORKEL SYSTEM Special Includes:
Only $185 (Plus Applicable Sales Tax) Note: As stated above, all students enrolling in OPEN WATER Classes are required to have an underwater timing device and underwater slate (or an acceptable dive computer) to keep track of dive times and other critical dive date. Any inexpensive water resistant wrist watch will suffice as the timing device and are available from most any discount department store starting in the price range of $20.00. Dive Slates are available in a wide variety of sizes and styles. We recommend one such as the model #SL61, which retails for $5.20, including an attached underwater pencil. All students are also required to own a SSI TOTAL DIVE LOG SYSTEM, which is where all training records are maintained as well as certification cards as they are earned. This log book includes a protective H.D. Zippered case to protect it and guarantee years of service and convenience. The SSI Total Dive Log System, #SSITDLS, is available from Underwater World Scuba/Sports or any authorized SSI (Scuba Schools International) Professional dive shop and sells for $49.00. |
